The Hallway Café Program offers a safe and supportive “go-to” place for at-risk youth. The Hallway Cafe offers employment readiness programming for youth as well as a source of program revenue for the agency from food and beverage services. This dual focused model is known as social enterprise.
As a Youth Case Coordinator, your primary responsibility is to provide support and case management to the youth participating in The Hallway Café Program.
Your role will focus on advocating and supporting youth in our program. You will work within a multi-disciplinary team to provide a full wrap around support model for youth in the program. You are responsible for ensuring each youth successfully navigates the progressive stages from outreach, into education, onto the employment readiness training, into job search, and ultimately into gainful employment or to transition into mainstream education.
A major focus is to oversee and develop youth case plans and walk alongside the youth in their progression to ensure alignment with youth goals and our programming outcomes.
In addition to responsibilities related to the youth case management and support, the Youth Case Coordinator is an integral part of a multi-disciplinary team designed to provide wrap around support to at-risk youth. At all times the Youth Case Coordinator is to role model and exemplify the skills taught throughout the program.
Education and Certifications
- Degree or Diploma in Child and Youth Care (CYC), Social Work, Human Services, or related field.
- Food safety training.
- Police Check with Vulnerable Sector.
- Intervention Record Check.
- Class 5 Driver’s License, reliable, registered vehicle, a recent driver’s abstract and valid insurance including two-million-dollar liability coverage.
- Minimum 2 years’ experience working with marginalized, high risk and vulnerable youth populations.
- Experience working in hospitality or food service an asset
- Experience working with youth experiencing addictions, poverty, mental health and trauma.
- Experience facilitating workshops.
- Working knowledge and experience with harm reduction, resiliency, trauma-informed and solution-focused practice.
- Understanding and the ability to respond to multiple, complex barriers faced by high-risk youth.
- Demonstrated experience de-escalating crisis situations with individuals under the influence of drugs or alcohol, under extreme stress &/or struggling with acute/chronic mental health crises.
- Build rapport with youth and be an active and supportive listener.
- Ensure youth clearly understand program expectations and are following through with these expectations.
- Build and maintain positive relationships with business partners and agencies within the community.
- Crisis intervention and one-on-one support to youth in partnership with multi-disciplinary team.
- Supporting youth with addictions, mental health, poverty, homelessness and PSECA concerns.
- Monitor the progression of the youth and assist the youth in transitioning into the employment readiness training when they are ready for that challenge.
- Monitor and assess the needs of the youth for supportive housing, addiction counseling, and other internal or external services; be a supportive bridge into these services.
- Communicate regularly with multi-disciplinary team regarding youth progress, significant changes in youth’s behaviour and/or any other concerns that the youth experiencing and share with the team members.
- Work with the youth to develop case plans.
- Support youth in developing goals. Follow up on set goals and progress with youth.
- Provide referral and follow-up support to external community services
- Conduct Casey Life Skill and Barrier assessments throughout the program to assess the progression in the youth’s development within the program.
- Provide opportunities and platforms that encourage self-advocacy in youth.
- Lead and facilitate multi-disciplinary meetings to discuss youth goals and growth.
- Completion of Critical Incident Reports (CIR) as required.
Transitional Youth Support
- Assist youth in determining the appropriate program streams (outreach, education, employment readiness).
- Support youth as they complete educational goals and connect youth to educational facilities in their community.
- Assist the youth in transitioning their newly developed skills into a new environment within both the practicum placement and job placement upon graduation.
- Conduct Casey Life Skills reassessment with youth.
- Support youth in job searches, resume writing and preparing for interviews.
- Engage with the youth while active in the program
- Engage in daily production tasks and customer service exercises within the café setting and within operational guidelines
- Maintain an understanding of the workflows, unique terminology & hospitality systems in place within the cafe
- Conduct workshops with youth including Life skills Learning, Stress Management, Food Service Training, and various modules in our Employment Training program.
- Adhere to Agency and Program Policies and Procedures in program delivery.
- Maintain, review and update youth files.
- Complete and submit other documentation as necessary.
- CANFIT entry and other reporting systems as required.
